With Carlos Alcaraz’s wrist injury dominating tennis headlines right now, it’s a timely reminder of just how vulnerable the wrist is in this sport — and how much is at stake when healing goes wrong.
In January 2026, she fell forward on court during a tennis trip to New Zealand — a classic FOOSH (fall on outstretched hand) mechanism — and sustained a scaphoid fracture. The scaphoid is a small carpal bone notorious for its precarious blood supply, its frequent misdiagnosis on plain X-ray, and its high risk of non-union if not managed carefully.
After approximately 8 weeks of conservative management, we began a course of focused Extracorporeal Shockwave Therapy (fESWT) combined with Extracorporeal Magnetotransduction Therapy (EMTT) on 12 March 2026 — 6 sessions in total, using the Storz Medical DUOLITH SD1 and MAGNETOLITH.

Why this combination? fESWT stimulates angiogenesis at the fracture site — elevating eNOS and VEGF within the first week, with new capillary formation confirmed from week 4. This is critical for the scaphoid’s proximal pole, which has a retrograde and inherently vulnerable blood supply. EMTT works simultaneously on osteoblastogenesis: peak RUNX2 and SP7 gene activation occurs within days 1–3 of treatment, driving collagen synthesis, ALP elevation, and accelerated hydroxyapatite deposition. Together they address the two core biological failures in difficult scaphoid healing — hypovascular bone and inadequate mineralisation.
Today, CT confirmed complete scaphoid union — solid trabecular bridging across the fracture site — at approximately 15 weeks from the start of treatment. She has been discharged. No surgery. No complications. Full function restored.
